UCHealth Patient Portal Login

1. Go to the mychart.uchealth.org
2. Enter My Health Connection Username and password
3. Click on the “Log in” button to access your UC Health account

Recover Health Connection Login Username

1. Click on the “Forgot Username” link
2. Enter your first name, last name, date of birth and ZIP Code
3. Click on the “Submit” button 

Note: Your My Health Connection login will be sent to you if we have your email address on file. Call the My Health Connection support line at 855-395-9031 to restore access to your account if you can’t recall any of this information or if you don’t have a My Chart working email address on file.

Reset Password

1. Click on the “Forgot Password” link
2. Enter your My Health Connection Username, date of birth and ZIP code
3. Click on the “Next” button
4. Keep following the rest of instructions to finish

Sign Up For A New Account

1. Click on the “Sign Up Now” button
2. Enter required information 
3. Click on the “Next” button
4. Continue to follow the rest of instruction to finish your registration
5. You can access your account once you’ve created your username and password.

The UC Health app for patients is available for download from the Google Play store or the Apple iTunes App Store. You must install the UCHealth App and search for your main healthcare organization in order to exchange information with it.

• Run the UCHealth patient application.
• Find your primary healthcare provider.
• then decide on a healthcare provider.
• Using the Switch Organizations button on the Mychart login screen or the Switch Organization menu option once you have logged in, you may quickly add or switch between your UCHealth accounts at other healthcare organizations.
